PARENTAL TRANSPORTATION RELEASE FORMS

1.  Pre-Trip Release Form
This form is for students who will be traveling to and/or from an event
by other means than school-provided transportation.
This signed form must be given to the Athletic Director
before the event,
e.g., for a Saturday trip the Athletic Director must have the form on Friday.

2.  On-Site Release Form
This form is for students who will be traveling home from an event
with parent(s)/guardian(s)
and may be given to the Athletic Director
or the coach as his representative
at the event.
